Filspari (Oral)
Sparsentan is used to help slow kidney function decline in adults with a kidney disease called primary immunoglobulin A nephropathy, who are at risk for their disease getting worse.
Sparsentan is available only under a special program. You must be registered in the program and understand the risks and benefits of this medicine.
Sparsentan may also be used for purposes not listed in this medication guide.

What is the most important information I should know about Filspari (Oral)?
Some drugs should not be used with sparsentan. Your treatment plan may change if you also use:
- aliskiren;
- medicine to treat high blood pressure in the lungs--macitentan, bosentan, ambrisentan and others; or
- heart or blood pressure medication--enalapril, candesartan, lisinopril, irbesartan, losartan, olmesartan, valsartan, telmisartan.
Tell your doctor if you have or have ever had:
- high blood pressure;
- heart problems; or
- liver or kidney disease.
You will need to have a negative pregnancy test before starting this treatment.
May cause birth defects. Do not use if you are pregnant. You must use effective birth control before starting treatment, during, and for at least 2 weeks after your last dose. Tell your doctor if you become pregnant.
Ask your doctor about the best form of birth control while you are using sparsentan.
Do not breastfeed while using this medicine
Side Effects
What are the side effects of Filspari (Oral)?
Get emergency medical help if you have signs of an allergic reaction: hives, difficult breathing, swelling of your face, lips, tongue, or throat.
Sparsentan may cause serious liver problems. Stop taking this medicine and call your doctor right away if you have nausea, vomiting, loss of appetite, stomach pain (upper right side), tiredness, weight loss, jaundice (yellowing of the skin or eyes), dark urine, fever, or itching.
Call your doctor at once if you have:
- a light-headed feeling, like you might pass out;
- kidney problems--swelling, urinating less, feeling tired or short of breath;
- high blood potassium--nausea, weakness, tingly feeling, chest pain, irregular heartbeats, loss of movement; or
- fluid retention--shortness of breath (even while lying down), swelling, rapid weight gain (especially in your face and midsection).
Your treatments may be changed, delayed, or permanently discontinued if you have certain side effects.
Common side effects may include:
- dizziness;
- swelling of hands, legs, ankles and feet; or
- anemia (low red blood cells).

Pregnancy & Breastfeeding
Can I take Filspari (Oral) if I’m pregnant or breastfeeding?
You will need to have a negative pregnancy test before starting this treatment.

Dosage Guidelines & Tips
How to take Filspari (Oral)?
Use Filspari (Oral) exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended.
Follow all directions on your prescription label and read all medication guides or instruction sheets. Your doctor may occasionally change your dose. Use the medicine exactly as directed.
Sparsentan is usually taken once per day. Follow your doctor's dosing instructions very carefully.
Swallow the tablet whole with water before your morning or evening meal. Take your dose before the same meal each day.
Drink plenty of liquids while you are taking sparsentan.
If you are at risk for low blood pressure, your doctor may adjust your other blood pressure medications.
Do not stop taking this medicine without your doctor's advice.
You will need frequent blood tests to check your liver function, before you start and every 3 months during treatment.
Your kidney function and blood levels of potassium may need to be checked often during treatment.
Store this medicine in the original container at room temperature away from moisture and heat.
Skip the missed dose and use your next dose at the regular time. Do not use two doses at one time.
Overdose Signs
What happens if I overdose on Filspari (Oral)?
What should I avoid while taking sparsentan?
Do not take potassium supplements or use salt substitutes, unless your doctor has told you to.
Grapefruit may interact with sparsentan and cause side effects. Ask your doctor before consuming grapefruit products.
Avoid taking a stomach acid reducer such as famotidine, cimetidine, ranitidine, omeprazole, pantoprazole, esomeprazole, and others.
Avoid taking an antacid within 2 hours before or after you take sparsentan, and take only the type your doctor recommends.
